I agree with your rule Steve. I do have another rule. If the respondent is supposed to circle a letter or number for their response, and the scale is for example 1, 2, 3, 4, 5, I will paste the numbers in after each question so that a pencil can be used to circle the appropriate response. I've done this on some surveys that are returned to people who are sighted. They can actually record the responses independently without needing to be able to read braille. Two of my clients have instructed us to add a Transcriber's Note that if the person who is blind has brailled comments on their survey that need to be read by someone who reads braille, the respondent is to write an X on any page that needs to be translated.

When I braille documents that have underlines like that, I just remove them and leave the rest of the line blank. That way, if someone is filling out the form by inserting the page in a braille writer, he/she will have space for the answer. I don't know if there's a rule for this, but I've just given you Steve's rule.
